Breakthru Health is in-network with the majority of major insurance carriers.  However, because insurance participation varies by therapist, please view each provider's profile on their website to verify that they accept your specific insurance plan before getting started.

Confirming Your Insurance Benefits

To verify your coverage, call the Member/Customer Service number on the back of your insurance card.

Have this information ready:

• Full Name & Date of Birth

• Insurance ID Number

• Social Security Number

 

Please Note:  You are ultimately responsible for verifying your coverage, obtaining necessary authorizations, and paying any deductibles, co-insurance, or copays. Verification is a helpful guide but is not a guarantee of coverage.
